1. Sponsor

Slice of Vegas Pizza Kitchen & Bar, The Shoppes at Mandalay Place, Las Vegas, NV (the "Sponsor").

2. Eligibility

• Open to legal residents of the United States who are 21 years of age or older at the time of entry.
• Employees of Sponsor, its affiliates, and members of their immediate families are not eligible.
• Void where prohibited or restricted by law.

3. Sweepstakes Period

The current "Season" begins at [START DATE / TIME PT] and ends at [END DATE / TIME PT]. Each Las Vegas Aces season is a separate sweepstakes period with its own drawing.

4. How to Enter

Online entry: Visit pizza4life.vegas, complete the form (first name, last name, mobile phone, optional email), check the SMS opt-in, and submit. One (1) entry per phone number per Season.

Alternative method of entry (AMOE) — no purchase, no SMS opt-in required: Mail a 3"x5" postcard with your full name, address, phone number, email (optional), and date of birth to: Slice of Vegas — WP4L AMOE, [SPONSOR MAILING ADDRESS]. Postcards must be received by [END DATE]. Limit one (1) AMOE entry per envelope and per person per Season.

5. Drawing & Winner Selection

One (1) winner will be selected at random from all eligible entries on or about [DRAWING DATE]. Selection will be conducted by Sponsor or Sponsor's designee using a random selection method.

6. Odds

Odds of winning depend on the total number of eligible entries received during the Sweepstakes Period.

7. Prize

• Description: "Free Pizza for Life" — defined as: [ONE FREE 12-INCH CHEESE PIZZA PER WEEK / OR: UNLIMITED FREE SLICES PER VISIT, ONE VISIT PER WEEK / OR: SLICE OF VEGAS TO DEFINE], redeemable in person at Slice of Vegas Pizza Kitchen & Bar, The Shoppes at Mandalay Place, Las Vegas, NV.
• Duration: [FOR THE LIFETIME OF THE WINNER / OR: 30 YEARS / OR: SO LONG AS THE RESTAURANT REMAINS OPEN, WHICHEVER IS SHORTER — SLICE OF VEGAS TO DEFINE].
• Approximate Retail Value (ARV): $[TO BE DETERMINED BY SPONSOR]. The winner is responsible for any applicable federal, state, and local taxes; Sponsor will issue an IRS Form 1099-MISC if the ARV exceeds $600.
• Restrictions: Non-transferable. Cannot be sold, exchanged, or substituted for cash. Cannot be combined with other offers. Subject to standard restaurant operating hours and availability. The winner must present valid photo ID at each redemption.
• If the restaurant ceases operations: The prize terminates without further obligation by Sponsor.
• Inheritability: The prize is not inheritable.

8. Winner Notification

The winner will be notified by SMS and/or phone using the contact information provided at entry, within [N] days of the drawing. The winner must respond and claim the prize within [N] days. If the winner cannot be reached or fails to respond, an alternate winner may be selected.

9. Publicity Release

By accepting the prize, the winner grants Sponsor permission to use the winner's name, city of residence, photograph, and likeness in promotional materials, without further compensation, except where prohibited by law.

10. General Release

By entering, you release and hold harmless Sponsor, its affiliates, and their employees from any liability arising from participation in the Sweepstakes or use of the prize.

11. Privacy

Information you provide is handled per our Privacy Policy.

12. Winners List

For the name of the winner, send a self-addressed stamped envelope after the drawing date to: Slice of Vegas — WP4L Winner, [SPONSOR MAILING ADDRESS].

13. Governing Law

This Sweepstakes is governed by the laws of the State of Nevada. Any dispute will be resolved in the state or federal courts located in Clark County, Nevada.
